Create the first admin account for Keyku.

Password rules

At least 12 characters.
Must not match the setup secret, username, or app name.
No placeholder such as admin, password, or changeme.

For simple installs, set ISHIKU_SETUP_SECRET in Docker Compose. As a hardened alternative, mount a Docker secret at /run/secrets/ishiku_setup_secret.

After setup, new users can request an account for admin approval.
